Diagnosis is primarily based on postoperative imaging findings rather than clinical symptoms or hemodynamic instability. In this case, the pulmonary artery catheter showed marked rightward displacement with abnormal axial rotation, suggesting displacement of the heart itself rather than an en bloc mediastinal shift due to lung atelectasis. Together with the presence of an unclosed pericardial defect, these findings supported the diagnosis of right-sided cardiac herniation.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/media.springernature.com\/lw685\/springer-static\/image\/art%3A10.1007%2Fs00134-026-08321-9\/MediaObjects\/134_2026_8321_Fig1_HTML.jpg?as=webp\" alt=\"\"\/><\/figure>\n\n\n\n<p>Although surgical repair is often considered, conservative management with close monitoring may be appropriate in selected patients. Postural intervention, especially placement in the left lateral decubitus position, can facilitate spontaneous reduction of the herniated heart.
