This study assessed whether deep learning applied to routine outpatient chest radiographs (CXRs) can identify individuals at high risk of incident COPD.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sectitle0020\">Methods<\/h3>\n\n\n\n<p>Using cancer screening trial data, we previously developed a convolutional neural network (CXR-Lung-Risk) to predict lung-related mortality from a CXR image. In this retrospective model validation study, we externally validated whether CXR-Lung-Risk was associated with incident COPD from routine CXRs. We identified outpatients without lung cancer, COPD, or emphysema who had a CXR taken from Jan 1, 2013, to Dec 31, 2014, at Massachusetts General or Brigham and Women\u2019s Hospitals in Boston, MA, USA. The primary outcome was 6-year incident COPD. Discrimination was assessed using area under the receiver operating characteristic curve (AUC) compared with the TargetCOPD clinical risk score. All analyses were stratified by smoking status. A secondary analysis was conducted in the Project Baseline Health Study (PBHS) to test associations between CXR-Lung-Risk with pulmonary function and plasma protein abundance. The PBHS study is registered with&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/\" target=\"_blank\" rel=\"noreferrer noopener\">ClinicalTrials.gov<\/a>,&nbsp;<a href=\"https:\/\/clinicaltrials.gov\/show\/NCT03154346\" target=\"_blank\" rel=\"noreferrer noopener\">NCT03154346<\/a>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sectitle0025\">Findings<\/h3>\n\n\n\n<p>The primary analysis consisted of data from 12\u2009550 ever-smokers (mean age 62\u00b74 years [SD 6\u00b78], 6135 [48\u00b79%] male, 6415 [51\u00b71%] female) and 15\u2009298 never-smokers (mean age 63\u00b70 years [8\u00b71], 6550 [42\u00b78%] male, 8748 [57\u00b72%] female). 1562 (12\u00b74%) of 12\u2009550 ever-smokers and 580 (3\u00b78%) of 15\u2009298 never-smokers developed COPD within 6 years. CXR-Lung-Risk had additive predictive value beyond the TargetCOPD score for 6-year incident COPD in both ever-smokers (CXR-Lung-Risk + TargetCOPD AUC 0\u00b773 [95% CI 0\u00b772\u20130\u00b774]\u00a0<em>vs<\/em>TargetCOPD alone AUC 0\u00b766 [0\u00b765\u20130\u00b768], p&lt;0\u00b70001) and never-smokers (CXR-Lung-Risk + TargetCOPD AUC 0\u00b770 [0\u00b767\u20130\u00b772]\u00a0<em>vs<\/em>\u00a0TargetCOPD alone AUC 0\u00b760 [0\u00b757\u20130\u00b762], p&lt;0\u00b70001).
