Bronchiectasis is a disease with a global impact, but most published data come from high-income countries. We aimed to describe the clinical characteristics of patients with bronchiectasis in China.<\/p>\n\n\n\n

\u80cc\u666f\uff1a\u652f\u6c14\u7ba1\u6269\u5f20\u75c7\u662f\u4e00\u79cd\u5728\u5168\u7403\u8303\u56f4\u5185\u5177\u6709\u91cd\u8981\u5f71\u54cd\u7684\u75be\u75c5\uff0c\u4f46\u5927\u591a\u6570\u5df2\u53d1\u8868\u7684\u6570\u636e\u6765\u81ea\u9ad8\u6536\u5165\u56fd\u5bb6\u3002\u672c\u7814\u7a76\u65e8\u5728\u63cf\u8ff0\u4e2d\u56fd\u652f\u6c14\u7ba1\u6269\u5f20\u75c7\u60a3\u8005\u7684\u4e34\u5e8a\u7279\u5f81\u56fe\u8c31\u3002<\/p>\n\n\n\n

Methods<\/h3>\n\n\n\n

The Chinese Bronchiectasis Registry (BE-China) is a prospective, observational cohort enrolling patients from 111 hospitals in China. Data on demographics, comorbidities, and aetiological testing results were collected from adult patients with bronchiectasis at baseline and annual follow-up. Patients who met the inclusion criteria (age \u226518 years; received chest high-resolution CT in the past year showing bronchiectasis affecting one or more lung lobes; and clinical history consistent with bronchiectasis, including chronic cough, daily sputum production, and history of exacerbations) were included. Patients with known cystic fibrosis were excluded. To investigate variations according to different economic regions, two groups were compared based on whether per capita disposable income of residents was greater than US$5553. Clinical characteristics were compared with the European (EMBARC) registry and other national registries.<\/p>\n\n\n\n

\u65b9\u6cd5\uff1a\u4e2d\u56fd\u652f\u6c14\u7ba1\u6269\u5f20\u75c7\u6ce8\u518c\u767b\u8bb0\u7814\u7a76\uff08BE-China\uff09\u662f\u4e00\u9879\u524d\u77bb\u6027\u3001\u89c2\u5bdf\u6027\u961f\u5217\u7814\u7a76\uff0c\u62db\u52df\u4e86\u6765\u81ea\u4e2d\u56fd 111 \u5bb6\u533b\u9662\u7684\u60a3\u8005\u3002\u7814\u7a76\u6536\u96c6\u6210\u5e74\u652f\u6c14\u7ba1\u6269\u5f20\u75c7\u60a3\u8005\u5728\u57fa\u7ebf\u548c\u5e74\u5ea6\u968f\u8bbf\u65f6\u7684\u4eba\u53e3\u7edf\u8ba1\u5b66\u3001\u5408\u5e76\u75c7\u548c\u75c5\u56e0\u5b66\u68c0\u6d4b\u7ed3\u679c\u7b49\u6570\u636e\u3002\u672c\u7814\u7a76\u7eb3\u5165\u7b26\u5408\u652f\u6269\u8bca\u65ad\u6807\u51c6\uff08\u5e74\u9f84\u226518 \u5c81\uff0c\u5728\u8fc7\u53bb\u4e00\u5e74\u4e2d\u63a5\u53d7\u8fc7\u80f8\u90e8\u9ad8\u5206\u8fa8\u7387 CT \u68c0\u67e5\uff0c\u663e\u793a\u4e00\u4e2a\u6216
\u591a\u4e2a\u80ba\u53f6\u5b58\u5728\u652f\u6c14\u7ba1\u6269\u5f20\uff0c\u4e34\u5e8a\u75c5\u53f2\u7b26\u5408\u652f\u6c14\u7ba1\u6269\u5f20\u75c7\uff0c\u6709\u6162\u6027\u54b3\u55fd\u3001\u6bcf\u65e5\u54b3\u75f0\u548c\u6025\u6027\u52a0\u91cd\u53f2\uff09\u7684\u60a3\u8005\u3002\u5df2\u77e5\u60a3\u6709\u56ca\u6027\u7ea4\u7ef4\u5316\u7684\u60a3\u8005\u88ab\u6392\u9664\u5728\u5916\u3002\u4e3a\u4e86\u7814\u7a76\u4e0d\u540c\u7ecf\u6d4e\u72b6\u51b5\u533a\u57df\u7684\u60a3\u8005\u7279\u5f81\u5dee\u5f02\uff0c\u57fa\u4e8e\u5c45\u6c11\u4eba\u5747\u53ef\u652f\u914d\u6536\u5165\u662f\u5426\u8d85\u8fc7 5553 \u7f8e\u5143\uff0c\u5c06\u60a3\u8005\u5206\u4e3a\u4e86\u4e24\u7ec4\u8fdb\u884c\u6bd4\u8f83\u3002\u540c\u65f6\uff0c\u4e2d\u56fd\u60a3\u8005\u4e34\u5e8a\u7279\u5f81\u4e0e\u6b27\u6d32\u652f\u6269\u6ce8\u518c\u767b\u8bb0\uff08EMBARC\uff09\u548c\u5176\u4ed6\u56fd\u5bb6\u7684\u6ce8\u518c\u767b\u8bb0\u7814\u7a76\u6570\u636e\u8fdb\u884c\u4e86\u6bd4\u8f83\u3002<\/p>\n\n\n\n

Findings<\/h3>\n\n\n\n

Between Jan 10, 2020, and March 31, 2024, 10\u2009324 patients from 97 centres were included in the study. Among 9501 participants with available data, the most common cause of bronchiectasis was post-infective disease (4101 [43\u00b72%] patients), followed by idiopathic (2809 [29\u00b76%] patients). 6676 (70\u00b70%) of 9541 patients with available data had at least one exacerbation in the year before enrolment and 5427 (57\u00b72%) of 9489 patients with available data were hospitalised at least once due to exacerbations. Treatments commonly used in high-income countries, such as inhaled antibiotics and macrolides, were infrequently used in China. Implementation of airway clearance in China was scarce, with only 1177 (12\u00b72%) of 9647 patients having used at least one method of airway clearance. Compared with upper-middle-income regions, patients from lower-middle-income regions were younger (61\u00b70 years [SD 14\u00b70]\u00a0vs<\/em>\u00a063\u00b79 years [14\u00b72]) with a higher proportion of pulmonary comorbidities (521 [17\u00b78%] of 2922 patients\u00a0vs<\/em>\u00a0639 [8\u00b76%] of 7402 with chronic obstructive pulmonary disease and 194 [6\u00b76%] of 2922 patients\u00a0vs<\/em>\u00a0364 [4\u00b79%] of 7402 patients with asthma), a higher tuberculosis burden (442 [16\u00b70%] of 2768 patients\u00a0vs<\/em>\u00a0715 [10\u00b76%] of 6733 patients), more severe radiological involvement (1160 [42\u00b74%] of 2736 patients\u00a0vs<\/em>\u00a02415 [35\u00b74%] of 6816 patients with cystic bronchiectasis), more exacerbations (median 1\u00b74 [IQR 0\u20132] in both groups; mean 1\u00b74 [SD 1\u00b76]\u00a0vs<\/em>\u00a01\u00b72 [1\u00b74] in the previous year) and hospitalisations (1662 [60\u00b76%] of 2743 patients\u00a0vs<\/em>\u00a03765 [55\u00b78%] of 6746 patients hospitalised at least once in the previous year), and poorer quality of life (median 57\u00b74 [IQR 53\u00b75\u201363\u00b71]\u00a0vs<\/em>\u00a058\u00b77 [54\u00b78\u201364\u00b78] assessed by the Bronchiectasis Health Questionnaire).<\/p>\n\n\n\n

\u7ed3\u679c\uff1a2020 \u5e74 1 \u6708 10 \u65e5\u81f3 2024 \u5e74 3 \u6708 31 \u65e5\u671f\u95f4\uff0c\u6700\u7ec8\u5171\u6709\u6765\u81ea 97 \u4e2a\u4e2d\u5fc3\u7684 10,324 \u540d\u60a3\u8005\u88ab\u7eb3\u5165\u7814\u7a76\u3002\u5728 9501 \u540d\u6709\u8db3\u591f\u6570\u636e\u7684\u60a3\u8005\u4e2d\uff0c\u652f\u6c14\u7ba1\u6269\u5f20\u75c7\u6700\u5e38\u89c1\u7684\u75c5\u56e0\u662f\u611f\u67d3\u540e\uff084101 \u4f8b\/43\u00b72%\uff09\uff0c\u5176\u6b21\u662f\u7279\u53d1\u6027\uff082809 \u4f8b\/29\u00b76%\uff09\u30026676 \u4f8b\uff0870\u00b70%\uff09\u60a3\u8005\u5728\u5165\u7ec4\u524d\u4e00\u5e74\u81f3\u5c11\u6709\u4e00\u6b21\u6025\u6027\u52a0\u91cd\uff0c5427 \u4f8b\uff0857\u00b72%\uff09\u60a3\u8005\u56e0\u6025\u6027\u52a0\u91cd\u81f3\u5c11\u4f4f\u9662\u6cbb\u7597\u8fc7\u4e00\u6b21\u3002\u5728\u9ad8\u6536\u5165\u56fd\u5bb6\u5e38\u7528\u7684\u6cbb\u7597\u65b9\u6cd5\uff0c\u5982\u5438\u5165\u6027\u6297\u751f\u7d20\u548c\u5927\u73af\u5185\u916f\u7c7b\u836f\u7269\u957f\u671f\u7ef4\u6301\u6cbb\u7597\uff0c\u5728\u4e2d\u56fd\u62a5\u9053\u4f7f\u7528\u8f83\u5c11\u3002\u6c14\u9053\u6e05\u9664\u6cbb\u7597\u5728\u4e2d\u56fd\u60a3\u8005\u7684\u5e94\u7528\u6bd4\u4f8b\u8f83\u4f4e\uff0c9647 \u540d\u60a3\u8005\u4e2d\u4ec5\u6709 1177 \u4f8b\uff0812\u00b72%\uff09\u8fdb\u884c\u8fc7\u81f3\u5c11\u4e00\u79cd\u6c14\u9053\u6e05\u9664\u6cbb\u7597\u3002\u4e0e\u4e2d\u9ad8\u6536\u5165\u5730\u533a\u76f8\u6bd4\uff0c\u6765\u81ea\u4e2d\u4f4e\u6536\u5165\u5730\u533a\u7684\u60a3\u8005\u66f4\u5e74\u8f7b\uff0861\u00b70

Interpretation<\/h3>\n\n\n\n

The clinical characteristics of patients with bronchiectasis in China show differences compared with cohorts in Europe and India. Bronchiectasis is more severe with a higher burden of exacerbations in lower-income regions. The management of patients with bronchiectasis in China urgently needs standardisation and improvement.<\/p>\n\n\n\n
