We aimed to provide such estimates for the period 1990\u20132021.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Methods<\/h3>\n\n\n\n<p>We estimated incidence, prevalence, death, and disability-adjusted life-year (DALY) counts and age-standardised rates per 100\u2008000 people per year for overall stroke, ischaemic stroke, intracerebral haemorrhage, and subarachnoid haemorrhage, for 204 countries and territories from 1990 to 2021. We also calculated burden of stroke attributable to 23 risk factors and six risk clusters (air pollution, tobacco smoking, behavioural, dietary, environmental, and metabolic risks) at the global and regional levels (21 GBD regions and Socio-demographic Index [SDI] quintiles), using the standard GBD methodology. 95% uncertainty intervals (UIs) for each individual future estimate were derived from the 2\u00b75th and 97\u00b75th percentiles of distributions generated from propagating 500 draws through the multistage computational pipeline.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Findings<\/h3>\n\n\n\n<p>In 2021, stroke was the third most common GBD level 3 cause of death (7\u00b73 million [95% UI 6\u00b76\u20137\u00b78] deaths; 10\u00b77% [9\u00b78\u201311\u00b73] of all deaths) after ischaemic heart disease and COVID-19, and the fourth most common cause of DALYs (160\u00b75 million [147\u00b78\u2013171\u00b76] DALYs; 5\u00b76% [5\u00b70\u20136\u00b71] of all DALYs). In 2021, there were 93\u00b78 million (89\u00b70\u201399\u00b73) prevalent and 11\u00b79 million (10\u00b77\u201313\u00b72) incident strokes. We found disparities in stroke burden and risk factors by GBD region, country or territory, and SDI, as well as a stagnation in the reduction of incidence from 2015 onwards, and even some increases in the stroke incidence, death, prevalence, and DALY rates in southeast Asia, east Asia, and Oceania, countries with lower SDI, and people younger than 70 years. Globally, ischaemic stroke constituted 65\u00b73% (62\u00b74\u201367\u00b77), intracerebral haemorrhage constituted 28\u00b78% (28\u00b73\u201328\u00b78), and subarachnoid haemorrhage constituted 5\u00b78% (5\u00b77\u20136\u00b70) of incident strokes. There were substantial increases in DALYs attributable to high BMI (88\u00b72% [53\u00b74\u2013117\u00b77]), high ambient temperature (72\u00b74% [51\u00b71 to 179\u00b75]), high fasting plasma glucose (32\u00b71% [26\u00b77\u201338\u00b71]), diet high in sugar-sweetened beverages (23\u00b74% [12\u00b77\u201335\u00b77]), low physical activity (11\u00b73% [1\u00b78\u201334\u00b79]), high systolic blood pressure (6\u00b77% [2\u00b75\u201311\u00b76]), lead exposure (6\u00b75% [4\u00b75\u201311\u00b72]), and diet low in omega-6 polyunsaturated fatty acids (5\u00b73% [0\u00b75\u201310\u00b75]).<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/ars.els-cdn.com\/content\/image\/1-s2.0-S1474442224003697-gr1a_lrg.jpg\" alt=\"\"\/><\/figure>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/ars.els-cdn.com\/content\/image\/1-s2.0-S1474442224003697-gr1b_lrg.jpg\" alt=\"\"\/><\/figure>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/ars.els-cdn.com\/content\/image\/1-s2.0-S1474442224003697-gr2_lrg.jpg\" alt=\"\"\/><\/figure>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/ars.els-cdn.com\/content\/image\/1-s2.0-S1474442224003697-gr3_lrg.jpg\" alt=\"\"\/><\/figure>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/ars.els-cdn.com\/content\/image\/1-s2.0-S1474442224003697-gr4_lrg.jpg\" alt=\"\"\/><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\">Interpretation<\/h3>\n\n\n\n<p>Stroke burden has increased from 1990 to 2021, and the contribution of several risk factors has also increased.
