{"id":23957,"date":"2023-06-03T04:53:00","date_gmt":"2023-06-02T20:53:00","guid":{"rendered":"http:\/\/csccm.org.cn\/?p=23957"},"modified":"2023-06-03T06:49:53","modified_gmt":"2023-06-02T22:49:53","slug":"lancet-infect-dis%e5%8f%91%e8%a1%a8%e8%bf%b0%e8%af%84%ef%bc%9a%e6%8c%87%e5%8d%97%e4%be%9d%e4%bb%8e%e6%80%a7%e6%98%af%e6%8f%90%e9%ab%98%e5%bf%b5%e7%8f%a0%e8%8f%8c%e8%a1%80%e7%97%87%e6%82%a3%e8%80%85","status":"publish","type":"post","link":"https:\/\/csccm.org.cn\/?p=23957","title":{"rendered":"[Lancet Infect Dis\u53d1\u8868\u8ff0\u8bc4]\uff1a\u6307\u5357\u4f9d\u4ece\u6027\u662f\u63d0\u9ad8\u5ff5\u73e0\u83cc\u8840\u75c7\u60a3\u8005\u751f\u5b58\u7387\u7684\u4e3b\u8981\u4fdd\u62a4\u63aa\u65bd"},"content":{"rendered":"\n<p>COMMENT|<a href=\"https:\/\/www.thelancet.com\/journals\/laninf\/onlinefirst\">ONLINE FIRST<\/a><\/p>\n\n\n\n<h1 class=\"wp-block-heading\">Guidelines adherence as a major protective aspect to increase survival rate in patients with candidaemia in Europe<\/h1>\n\n\n\n<h3 class=\"wp-block-heading\">Mohammad Sadegh Rezai, Fatemeh Ahangarkani<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">Lancet Infect Dis Published:February 15, 2023<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">DOI:<a href=\"https:\/\/doi.org\/10.1016\/S1473-3099(23)00046-4\">https:\/\/doi.org\/10.1016\/S1473-3099(23)00046-4<\/a><\/h3>\n\n\n\n<p id=\"para10\">Despite improvements in medical interventions and new&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/antifungal-agent\">antifungal drugs<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/candidemia\">candidaemia<\/a>is widely recognised as a major cause of morbidity and mortality worldwide.<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib1\">1<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib2\">2<\/a>&nbsp;Notably, the shift from typical candidaemia due to&nbsp;<em><a href=\"https:\/\/www.sciencedirect.com\/topics\/immunology-and-microbiology\/candida-albicans\">Candida albicans<\/a><\/em>&nbsp;towards candidaemia caused by various other&nbsp;<em>Candida<\/em>&nbsp;spp is associated with increased mortality and reduced susceptibility to antifungal drugs.<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib3\"><sup>3<\/sup><\/a>&nbsp;Recommendations for diagnosing and managing candidaemia can be found in the European Society of&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/immunology-and-microbiology\/clinical-microbiology\">Clinical Microbiology<\/a>&nbsp;and Infectious Diseases (ESCMID) and the Infectious Diseases Society of America (IDSA) guidelines.<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib4\">4<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib5\">5<\/a><\/p>\n\n\n\n<p id=\"para20\">In a multicentre study published in&nbsp;<em>The Lancet Infectious Diseases<\/em>, Martin Hoenigl and colleagues<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib6\"><sup>6<\/sup><\/a>&nbsp;provide data on&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/cohort-effect\">epidemiology<\/a>, risk factors, treatment, and outcomes of patients with culture-proven&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/candidemia\">candidaemia<\/a>&nbsp;across Europe to assess how adherence to candidaemia guideline recommendations is associated with outcomes. The authors assessed adherence to the&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/immunology-and-microbiology\/clinical-microbiology\">ESCMID<\/a>&nbsp;and the IDSA guidelines using the European Confederation of&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/medical-mycology\">Medical Mycology<\/a>&nbsp;Quality of Clinical Candidaemia Management (EQUAL&nbsp;<em>Candida<\/em>) score.<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib6\"><sup>6<\/sup><\/a>&nbsp;Higher EQUAL&nbsp;<em>Candida<\/em>&nbsp;scores reflect increased adherence to clinical guideline suggestions.<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib7\"><sup>7<\/sup><\/a><\/p>\n\n\n\n<p id=\"para30\">The study included 632 patients with candidaemia from 64 institutions in 20 European countries recruited after July 1, 2018. The majority of patients (58%) were male, and the median age was 65 years (IQR 53\u201373). The most common underlying conditions were haematological or oncological malignancy (39% of patients), intensive care unit (ICU) admission (37%), and recent major surgery (26%). Catheter-related candidaemia was observed in 21% of patients.&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/echocardiography\">Echocardiography<\/a>&nbsp;was done in 35% of patients, showing cardiac involvement in 11%, whereas&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/ophthalmoscopy\">ophthalmoscopy<\/a>&nbsp;was done in 27% of patients, showing ocular involvement in 11%.&nbsp;<em><a href=\"https:\/\/www.sciencedirect.com\/topics\/immunology-and-microbiology\/candida-albicans\">C albicans<\/a><\/em>&nbsp;(46%),&nbsp;<em>C glabrata<\/em>&nbsp;(21%),&nbsp;<em>C parapsilosis<\/em>&nbsp;(13%), and&nbsp;<em>C tropicalis<\/em>&nbsp;(7%) were the most commonly isolated&nbsp;<em>Candida<\/em>&nbsp;spp. Emerging&nbsp;<em>Candida<\/em>spp were less common (eg,&nbsp;<em>C auris<\/em>&nbsp;in 3% of patients). Initial&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/echinocandin\">echinocandin<\/a>&nbsp;treatment was the first-line&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/antifungal-therapy\">antifungal treatment<\/a>&nbsp;in 56% of patients, and step-down therapy to&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/fluconazole\">fluconazole<\/a>&nbsp;was given to 29% of patients. The all-cause (overall) mortality was 46%, of which 37% was attributable to candidaemia, and the overall 90-day mortality was 43%.<\/p>\n\n\n\n<p id=\"para40\">Hoenigl and colleagues<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib6\"><sup>6<\/sup><\/a>&nbsp;reported that the EQUAL&nbsp;<em>Candida<\/em>&nbsp;scores were associated with the duration of hospital stay (p&lt;0\u00b70001), and even after excluding patients hospitalised for 7 days or less, the EQUAL&nbsp;<em>Candida<\/em>&nbsp;scores were higher in survivors than in non-survivors (p&lt;0\u00b70001). In the&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/proportional-hazards-model\">Cox regression<\/a>&nbsp;analysis, increasing age, ICU admission, point increases in the&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/charlson-comorbidity-index\">Charlson comorbidity index<\/a>&nbsp;score, and&nbsp;<em>C tropicalis<\/em>&nbsp;as causative&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/pathogen\">pathogen<\/a>were independent baseline predictors of mortality.<\/p>\n\n\n\n<p id=\"para50\">Moreover, after adjusting for the baseline predictors in the multivariable&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/proportional-hazards-model\">Cox regression<\/a>analyses, the EQUAL&nbsp;<em>Candida<\/em>&nbsp;score remained an independent predictor of mortality. The authors stated that mortality in patients for whom guideline-recommended diagnostic or therapeutic measures were not performed was higher (51\u201371%) than mortality in the overall cohort (46%). In the multivariable Cox model comparing the effect of guideline-recommended diagnostic or therapeutic measures for patients who survived for more than 7 days and adjusted for the baseline predictors, the following not-performed or not-completed measures were predictors of mortality:&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/ophthalmoscopy\">ophthalmoscopy<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/echocardiography\">echocardiography<\/a>, treatment for 14 days or more after the first negative blood culture, and step-down therapy to&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/fluconazole\">fluconazole<\/a>.<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib6\"><sup>6<\/sup><\/a><\/p>\n\n\n\n<p id=\"para60\">Hoenigl and colleagues comprehensively described how adherence to guidelines might be promising for the survival of patients with candidaemia. The authors found that initial&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/echinocandin\">echinocandin<\/a>&nbsp;treatment was associated with reduced overall mortality and increased hospital stay among survivors despite the possibility of associated risks, such as hospital-acquired infections and increased costs. Notably, although treatment with echinocandins in patients with candidaemia has higher efficacy than treatment with other&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/antifungal-agent\">antifungals<\/a>,<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib8\"><sup>8<\/sup><\/a>the emergence of pan-echinocandin-resistant&nbsp;<em>Candida<\/em>&nbsp;spp should be considered.<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib9\"><sup>9<\/sup><\/a>Additionally, in some situations, echinocandins are not an appropriate treatment option for the empirical treatment of candidaemia. For example, in patients in whom candidaemia has a renal origin, fluconazole is preferable to other antifungal drugs, whereas in cases of suspected&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/topics\/medicine-and-dentistry\/mycosis\">fungal infection<\/a>&nbsp;and absence of microbiological diagnosis results (due to the lack of access to appropriate laboratory facilities and equipment in low-income countries<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1473309923000464#bib10\"><sup>10<\/sup><\/a>), broad-spectrum antifungals should be used instead of echinocandins.<\/p>\n\n\n\n<p id=\"para70\">The importance of the study by Hoenigl and colleagues originates from several factors. First, the study, which was done on a large scale across Europe, showed that overall 90-day mortality was high in European countries, suggesting that candidemia remains a medical emergency. Second, the study clearly showed the importance of individual, guideline-recommended diagnostic and therapeutic measures in the successful management of candidaemia.
