The press release stated that molnupiravir, a nucleoside analogue that inhibits viral replication by mutagenesis, reduced risk of hospital admission or death by about 50% (P=0.0012) in the 29 days after infection.<\/p>\n\n\n\n<p id=\"p-4\">The UK Medicines and Healthcare Products Regulatory Agency (MHRA) gave molnupiravir conditional marketing authorisation on 4 November 2021, based on the interim data underlying the press release.<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-2\">2<\/a>&nbsp;On 23 December, the US Food and Drug Administration (FDA) granted emergency use authorisation after seeing the trial\u2019s full dataset.<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-3\">3<\/a>These authorisations, conducted behind closed doors, lack adequate scientific rigour, inhibit further necessary evaluations, and may ultimately lead to suboptimal resource allocation decisions.<\/p>\n\n\n\n<p id=\"p-5\">Both emergency use and conditional marketing authorisations have a lower bar for efficacy than standard approvals. The language in the FDA\u2019s press release makes this clear, stating: \u201cThe agency has determined that it is reasonable to believe that molnupiravir may be effective.\u201d<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-3\">3<\/a>&nbsp;This raises questions about the strength and certainty of the evidence. The same lower standards underly the FDA\u2019s controversial accelerated approvals of aducanumab for Alzheimer\u2019s disease and eteplirsen for muscular dystrophy.<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-4\">4<\/a><a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-5\">5<\/a>&nbsp;The revolving door between regulatory employees and leading drug companies raises further concerns about conflicts of interest and scientific objectivity in regulatory decision making.<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-6\">6<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Dangers of truncated trials<\/h2>\n\n\n\n<p id=\"p-6\">Move-Out was stopped prematurely for reasons that aren\u2019t completely clear. When the trial was published, the authors reported a smaller absolute difference in primary outcome (\u22123%, 95% confidence interval \u22125.9 to \u22120.1, P=0.043)<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-7\">7<\/a>&nbsp;than quoted in Merck\u2019s earlier press release (\u22126.8%, P=0.0012). As the upper limit of this 95% confidence interval approaches zero, these published results have borderline significance, indicating that even a small number of misclassified outcomes could overturn the significance of the findings.<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-8\">8<\/a>&nbsp;Robustness is further undermined by a modified intention-to-treat analysis that excluded 25 patients after randomisation. Whether a standard intention-to-treat analysis of all randomised patients would have produced significant results remains unclear.<\/p>\n\n\n\n<p id=\"p-7\">The weakness of the evidence supporting molnupiravir has been partially obscured by the large relative benefit reported in Merck\u2019s original press release. This created a cognitive bias by anchoring public and scientific opinion to a belief in substantial benefit. Press releases can also set unreasonable expectations among the public and policy makers, leading to demand for immediate access to poorly evaluated drugs.<\/p>\n\n\n\n<p id=\"p-8\">Good evidence exists that prematurely terminated trials are more likely than non-truncated trials to overestimate effect sizes. In one meta-analysis, 91 truncated trials reported greater effect sizes than 424 matched non-truncated trials,<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-9\">9<\/a>&nbsp;independent of statistical stopping rules. Smaller truncated trials with less than 200 events (Move-Out had 112 events) reported a 63% greater reduction in relative risk than similar trials that were not truncated. Interestingly, Move-Out\u2019s truncated relative risk reduction (0.5)<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-1\">1<\/a>&nbsp;is 66% higher than the non-truncated relative risk reduction (0.3),<sup>7<\/sup>&nbsp;emphasising the dangers of making decisions based on a single prematurely terminated trial.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Clinical value?<\/h2>\n\n\n\n<p id=\"p-9\">The real question for any trial is whether the findings are clinically as well as statistically significant. There are two main challenges in answering that question: firstly, determining how best to define clinical significance and, secondly, the fact that null hypothesis significance testing is inadequate for evaluating the clinical relevance of an effect. As Move-Out was powered to detect an absolute reduction of 6% in hospital admissions or death, we might reasonably accept this as the minimum clinically meaningful difference. The confidence interval around the published reduction in primary outcome (\u22123%, 95% CI \u22125.9 to \u22120.1)<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-7\">7<\/a>&nbsp;excludes 6%, leading to questions about the clinical importance of these findings.<\/p>\n\n\n\n<p id=\"p-10\">Molnupiravir\u2019s safety profile is also uncertain because Move-Out was underpowered, like most clinical trials, to detect clinically important side effects. Its mutagenic mode of action could in theory encourage emergence of additional SARS-CoV-2 variants. The generalisability of the results is also unclear as the trial population was unvaccinated and recruited before the most recent variants emerged, with 70% from low and middle income countries. It is not known whether reported benefits will be maintained across different populations with differing variants, ancillary treatments, and healthcare systems. Further uncertainty arises from two more recent studies of molnupiravir reporting no clinical benefit in either outpatients or inpatients with covid-19.<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-10\">10<\/a><a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-11\">11<\/a><\/p>\n\n\n\n<p id=\"p-11\">The importance of clinical benefit is highlighted by molnupiravir\u2019s high costs. Five days\u2019 treatment is expected to cost about $700 (\u00a3500; \u20ac600), and with the manufacturer estimating that demand will reach 10 million courses over the next year,<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-1\">1<\/a>&nbsp;global costs could reach several billion dollars annually. The opportunity costs associated with this agent, including the potential for more financial support for mass vaccinations in low and middle income countries, merit serious reflection.<\/p>\n\n\n\n<p id=\"p-12\">Detailed cost effectiveness studies by independent health technology assessment groups must be done to avoid repeating past mistakes. Approval of oseltamivir (Tamiflu), an antiviral for early uncomplicated influenza, resulted in worldwide governmental spending exceeding $18bn (half on stockpiling) for a drug with no benefit other than reducing duration of symptoms by less than one day.<a href=\"https:\/\/www.bmj.com\/content\/376\/bmj.o443?ijkey=4fb553152b5befc22fdb1eb5f94a935708e0a073&amp;keytype2=tf_ipsecsha#ref-12\">12<\/a><\/p>\n\n\n\n<p id=\"p-13\">The evidence for incorporating molnupiravir into routine practice is fragile. Premature regulatory authorisation and guideline recommendation<sup>13<\/sup>\u00a0on the basis of truncated and non-replicated trial findings and without full consideration of clinical, not statistical, significance and cost effectiveness falls far short of the wise stewardship of limited healthcare resources required during a global healthcare emergency. We deserve and should demand better.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>Letters<\/strong>\u00a0Molnupiravir\u2019s premature authorisation<\/p>\n\n\n\n<h1 class=\"wp-block-heading\" id=\"page-title\">Molnupiravir\u2019s authorisation should be re-evaluated after the Panoramic trial is reported<\/h1>\n\n\n\n<h3 class=\"wp-block-heading\">Andrew Hill<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\"><em>BMJ<\/em>\u00a02022;\u00a0377\u00a0doi:\u00a0<a href=\"https:\/\/doi.org\/10.1136\/bmj.o973\">https:\/\/doi.org\/10.1136\/bmj.o973<\/a>\u00a0(Published 26 April 2022)Cite this as:\u00a0<em>BMJ<\/em>\u00a02022;377:o973<\/h3>\n\n\n\n<p id=\"p-2\">Brophy\u2019s editorial on Merck\u2019s Move-Out trial of molnupiravir raises important concerns about the approval of molnupiravir to treat outpatients with covid-19 in the United Kingdom.<a href=\"https:\/\/www.bmj.com\/content\/377\/bmj.o973#ref-1\">1<\/a>&nbsp;Molnupiravir has not yet been approved in Europe,<a href=\"https:\/\/www.bmj.com\/content\/377\/bmj.o973#ref-2\">2<\/a>&nbsp;and orders were recently cancelled in France.<a href=\"https:\/\/www.bmj.com\/content\/377\/bmj.o973#ref-3\">3<\/a><\/p>\n\n\n\n<p id=\"p-3\">Molnupiravir reduced the risk of hospital admission or death by 52% versus placebo in the interim analysis but raised risk by 35% among patients subsequently evaluated.<a href=\"https:\/\/www.bmj.com\/content\/377\/bmj.o973#ref-4\">4<\/a>&nbsp;There was no clear explanation for this substantial disparity in treatment effects between the interim and final phases. Molnupiravir showed no overall benefit over placebo for resolution of covid-19 related signs and symptoms.<a href=\"https:\/\/www.bmj.com\/content\/377\/bmj.o973#ref-5\">5<\/a><\/p>\n\n\n\n<p id=\"p-4\">Molnupiravir is now being evaluated in the UK Panoramic trial, coordinated by Oxford University (<a href=\"https:\/\/www.panoramictrial.org\/\">https:\/\/www.panoramictrial.org\/<\/a>). The inclusion criteria and primary efficacy endpoint are similar to the Move-Out trial, with two exceptions. Panoramic includes mainly vaccinated patients, and is being conducted during the Omicron wave, whereas Move-Out included only unvaccinated patients and was conducted in 2021, when previous variants led to more severe disease.<\/p>\n\n\n\n<p id=\"p-5\">In Panoramic, the original sample size calculation of 10\u2009600 patients assumed a 3% rate of hospital admission or death for standard of care, and 2% for patients taking molnupiravir. By 1 April 2022, Panoramic had enrolled 21\u2009460 patients, over double the original estimate. This suggests that rates of hospital admission or death could be even lower than originally predicted. So the clinical benefits of molnupiravir and associated cost-effectiveness might be lower for vaccinated patients in the current Omicron wave.<\/p>\n\n\n\n<p id=\"p-6\">As Brophy explains,<a href=\"https:\/\/www.bmj.com\/content\/377\/bmj.o973#ref-1\">1<\/a>&nbsp;the Move-Out trial was terminated after the interim analysis for ethical reasons, after only 762 patients had been evaluated. By contrast, the Panoramic trial, with the same inclusion criteria and primary endpoint but with a sample size 27 times larger, is still going.<\/p>\n\n\n\n<p id=\"p-7\">The UK approval of molnupiravir should be re-evaluated after results from the Panoramic trial have been reported.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>Letters<\/strong>&nbsp;Living WHO guideline on molnupiravir for covid-19<\/p>\n\n\n\n<h1 class=\"wp-block-heading\" id=\"page-title\">Molnupiravir might be dangerous without clarification of its indications<\/h1>\n\n\n\n<h3 class=\"wp-block-heading\">Keiji Hayashi<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\"><em>BMJ<\/em>\u00a02022;\u00a0377\u00a0doi:\u00a0<a href=\"https:\/\/doi.org\/10.1136\/bmj.o1030\">https:\/\/doi.org\/10.1136\/bmj.o1030<\/a>\u00a0(Published 26 April 2022)Cite this as:\u00a0<em>BMJ<\/em>\u00a02022;377:o1030<\/h3>\n\n\n\n<p id=\"p-2\">The data on which the approval of molnupiravir was based in Japan<a href=\"https:\/\/www.bmj.com\/content\/377\/bmj.o1030#ref-1\">1<\/a>&nbsp;and the data reported by Bernal et al<a href=\"https:\/\/www.bmj.com\/content\/377\/bmj.o1030#ref-2\">2<\/a>&nbsp;are worrying and emphasise the huge unknowns around the use of molnupiravir.<a href=\"https:\/\/www.bmj.com\/content\/377\/bmj.o1030#ref-3\">3<\/a><\/p>\n\n\n\n<p id=\"p-3\">The first problem is efficacy estimates that change substantially over time. In the interim analysis, the relative risk (RR) for hospital admission for any cause or death through day 29 was 0.52 (95% confidence interval 0.34 to 0.80). But the effect was less strong in the final analysis (RR 0.70 (0.49 to 0.99)).<\/p>\n\n\n\n<p id=\"p-4\">A bigger problem is that the RR after interim analysis was 1.33 (0.69 to 2.54). The 95% confidence intervals for the before and after interim analysis overlap only slightly, and a significant difference between them cannot be ruled out. Moreover, the direction of the effect is different before and after the interim analysis. In the interim analysis, molnupiravir reduced the primary outcome, but afterwards it tended to increase it.<\/p>\n\n\n\n<p id=\"p-5\">The rate of hospital admission or death for placebo is different between the two: 53 out of 377 patients in the interim analysis and 15 out of 322 after the interim analysis (RR 3.02 (1.74 to 5.25)). This alone indicates that the difference is most likely due to the different demographics of the two groups of participants.<\/p>\n\n\n\n<p id=\"p-6\">If this is the case, then instead of combining the interim and later analyses, we should analyse the differences in background and identify the groups that benefit and those that are harmed.
