The efficacy of extracorporeal membrane oxygenation (ECMO) for patients with severe ARDS in the context of COVID-19 is unclear. We aimed to establish the clinical characteristics and outcomes of patients with respiratory failure and COVID-19 treated with ECMO.<\/p>\n\n\n\n<p>\u53d1\u751f\u91cd\u5ea6ARDS\u7684\u65b0\u51a0\u80ba\u708e\u60a3\u8005\u4e34\u5e8a\u75c7\u72b6\u8fc5\u901f\u8fdb\u5c55\u4e3a\u4e25\u91cd\u4f4e\u6c27\u8840\u75c7\u751a\u81f3\u6b7b\u4ea1\u3002ECMO\u5bf9\u4e8e\u65b0\u51a0\u80ba\u708e\u5bfc\u81f4\u7684ARDS\u60a3\u8005\u7684\u7597\u6548\u5e76\u4e0d\u660e\u786e\u3002\u6211\u4eec\u65e8\u5728\u660e\u786e\u63a5\u53d7ECMO\u6cbb\u7597\u5408\u5e76\u547c\u5438\u529f\u80fd\u8870\u7aed\u7684\u65b0\u51a0\u80ba\u708e\u60a3\u8005\u7684\u4e34\u5e8a\u7279\u5f81\u53ca\u9884\u540e\u3002<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Methods \u65b9\u6cd5<\/h3>\n\n\n\n<p>This retrospective cohort study was done in the Paris\u2013Sorbonne University Hospital Network, comprising five intensive care units (ICUs) and included patients who received ECMO for COVID-19 associated ARDS. Patient demographics and daily pre-ECMO and on-ECMO data and outcomes were collected. Possible outcomes over time were categorised into four different states (states 1\u20134): on ECMO, in the ICU and weaned off ECMO, alive and out of ICU, or death. Daily probabilities of occupation in each state and of transitions between these states until day 90 post-ECMO onset were estimated with use of a multi-state Cox model stratified for each possible transition. Follow-up was right-censored on July 10, 2020.<\/p>\n\n\n\n<p>\u8fd9\u9879\u56de\u987e\u961f\u5217\u7814\u7a76\u5728Paris\u2013Sorbonne\u5927\u5b66\u533b\u9662\u7f51\u7edc\u8fdb\u884c\uff0c5\u4e2aICU\u53c2\u52a0\uff0c\u7eb3\u5165\u63a5\u53d7ECMO\u6cbb\u7597\u7684\u65b0\u51a0\u80ba\u708e\u5408\u5e76ARDS\u60a3\u8005\u3002\u6536\u96c6\u60a3\u8005\u4eba\u53e3\u7edf\u8ba1\u5b66\u8d44\u6599\uff0c\u5df2\u7ecfECMO\u524d\u548c\u5e94\u7528ECMO\u8fc7\u7a0b\u4e2d\u6bcf\u65e5\u8d44\u6599\u548c\u9884\u540e\u3002\u6839\u636e\u53ef\u80fd\u7684\u65f6\u95f4\u76f8\u5173\u9884\u540e\u5206\u4e3a4\u79cd\u4e0d\u540c\u72b6\u6001\uff08\u72b6\u6001 1\u20134\uff09\uff1a\u4f7f\u7528ECMO\u8fc7\u7a0b\u4e2d\uff0c\u4ecd\u5728 ICU\u4f46\u8131\u79bb ECMO\uff0c\u5b58\u6d3b\u4e14\u8f6c\u51faICU\uff0c\u6216\u6b7b\u4ea1\u3002\u91c7\u7528\u591a\u72b6\u6001Cox\u6a21\u578b\uff0c\u9488\u5bf9\u6bcf\u79cd\u53ef\u80fd\u8f6c\u53d8\u5206\u5c42\uff0c\u8ba1\u7b97\u6bcf\u65e5\u5728\u5404\u79cd\u72b6\u6001\u53ca\u4e4b\u95f4\u8f6c\u53d8\u7684\u6982\u7387\u3002\u968f\u8bbf\u622a\u6b62\u52302020\u5e747\u670810\u65e5\u3002<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Findings \u7ed3\u679c<\/h3>\n\n\n\n<p>From March 8 to May 2, 2020, 492 patients with COVID-19 were treated in our ICUs. Complete day-60 follow-up was available for 83 patients (median age 49 [IQR 41\u201356] years and 61 [73%] men) who received ECMO. Pre-ECMO, 78 (94%) patients had been prone-positioned; their median driving pressure was 18 (IQR 16\u201321) cm H<sub>2<\/sub>O and PaO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0was 60 (54\u201368) mm Hg. At 60 days post-ECMO initiation, the estimated probabilities of occupation in each state were 6% (95% CI 3\u201314) for state 1, 18% (11\u201328) for state 2, 45% (35\u201356) for state 3, and 31% (22\u201342) for state 4. 35 (42%) patients had major bleeding and four (5%) had a haemorrhagic stroke. 30 patients died.<\/p>\n\n\n\n<p>\u4ece2020\u5e743\u67088\u65e5\u81f35\u67082\u65e5\uff0cICU\u5171\u6536\u6cbb492\u540d\u65b0\u51a0\u80ba\u708e\u60a3\u8005\u3002\u5176\u4e2d83\u540d\uff08\u4e2d\u4f4d\u5e74\u9f84 49 [IQR 41\u201356] \u5c81\uff0c61\u540d [73%] \u4e3a\u5973\u6027\uff09\u60a3\u8005\u6709\u5b8c\u6574\u768460\u5929\u968f\u8bbf\u8bb0\u5f55\u3002\u5728ECMO\u524d\uff0c78\u540d (94%) \u60a3\u8005\u63a5\u53d7\u4fef\u5367\u4f4d\uff1b\u9a71\u52a8\u538b\u4e2d\u4f4d\u657018 (IQR 16\u201321) cm H<sub>2<\/sub>O\uff0cPaO<sub>2<\/sub>\/FiO<sub>2<\/sub>\u00a0\u4e3a 60 (54\u201368) mm Hg\u3002\u5f00\u59cbECMO\u6cbb\u7597\u540e60\u5929\uff0c\u4f30\u6d4b\u5728\u6bcf\u79cd\u72b6\u6001\u7684\u6982\u7387\u5206\u522b\u662f\u72b6\u60011\u4e3a 6% (95% CI 3\u201314)\uff0c\u72b6\u60012\u4e3a18% (11\u201328)\uff0c\u72b6\u60013\u4e3a 45% (35\u201356)\uff0c\u72b6\u60014\u4e3a 31% (22\u201342)\u300235\u540d (42%) \u60a3\u8005\u6709\u5927\u51fa\u8840\uff0c4\u540d\u60a3\u8005 (5%) \u53d1\u751f\u51fa\u8840\u6027\u5352\u4e2d\u300230\u540d\u60a3\u8005\u6b7b\u4ea1\u3002<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/els-jbs-prod-cdn.jbs.elsevierhealth.com\/cms\/attachment\/40c3c049-b400-43c7-b05a-ad602a6e12d4\/gr1.gif\" alt=\"\"\/><\/figure>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/els-jbs-prod-cdn.jbs.elsevierhealth.com\/cms\/attachment\/a3e1bf66-8312-4369-b7f6-d657c44a413b\/gr2.jpg\" alt=\"\"\/><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\">Interpretation \u7ed3\u8bba<\/h3>\n\n\n\n<p>The estimated 60-day survival of ECMO-rescued patients with COVID-19 was similar to that of studies published in the past 2 years on ECMO for severe ARDS.
