The benefit of preventive short-term antibiotic therapy has not been shown.<\/p>\n\n\n\n<p>\u56e0\u53ef\u9664\u98a4\u5fc3\u5f8b\u5bfc\u81f4\u9662\u5916\u5fc3\u8df3\u9aa4\u505c\u4e14\u63a5\u53d7\u76ee\u6807\u6027\u4f53\u6e29\u7ba1\u7406\u7684\u60a3\u8005\u53d1\u751f\u547c\u5438\u673a\u76f8\u5173\u80ba\u708e\u7684\u98ce\u9669\u589e\u52a0\u3002\u77ed\u671f\u9884\u9632\u6027\u6297\u751f\u7d20\u7684\u76ca\u5904\u5c1a\u4e0d\u660e\u786e\u3002<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">METHODS \u65b9\u6cd5<\/h2>\n\n\n\n<p>We conducted a multicenter, double-blind, randomized, placebo-controlled trial involving adult patients (>18 years of age) in intensive care units (ICUs) who were being mechanically ventilated after out-of-hospital cardiac arrest related to initial shockable rhythm and treated with targeted temperature management at 32 to 34\u00b0C. Patients with ongoing antibiotic therapy, chronic colonization with multidrug-resistant bacteria, or moribund status were excluded. Either intravenous amoxicillin\u2013clavulanate (at doses of 1 g and 200 mg, respectively) or placebo was administered three times a day for 2 days, starting less than 6 hours after the cardiac arrest. The primary outcome was early ventilator-associated pneumonia (during the first 7 days of hospitalization). An independent adjudication committee determined diagnoses of ventilator-associated pneumonia.<\/p>\n\n\n\n<p>\u6211\u4eec\u8fdb\u884c\u4e86\u4e00\u9879\u591a\u4e2d\u5fc3\u3001\u53cc\u76f2\u3001\u968f\u673a\u3001\u5b89\u6170\u5242\u5bf9\u7167\u8bd5\u9a8c\uff0c\u5165\u9009\u56e0\u53ef\u9664\u98a4\u5fc3\u5f8b\u5bfc\u81f4\u9662\u5916\u5fc3\u8df3\u9aa4\u505c\u7684\u6210\u5e74 (>18 \u5c81)\u60a3\u8005\uff0c\u6536\u5165ICU\uff0c\u63a5\u53d7\u76ee\u6807\u4f53\u6e29(32 to 34\u00b0C)\u7ba1\u7406\u4e14\u5e94\u7528\u673a\u68b0\u901a\u6c14\u3002\u5df2\u63a5\u53d7\u6297\u751f\u7d20\u6cbb\u7597\u3001\u591a\u91cd\u8010\u836f\u83cc\u957f\u671f\u5b9a\u690d\u6216\u4e34\u7ec8\u72b6\u6001\u60a3\u8005\u6392\u9664\u3002\u5728\u5fc3\u8df3\u9aa4\u505c\u540e6\u5c0f\u65f6\u5185\uff0c\u60a3\u8005\u5206\u522b\u63a5\u53d7\u9759\u8109\u963f\u83ab\u897f\u6797-\u514b\u62c9\u7ef4\u9178(\u5242\u91cf\u5206\u522b\u4e3a1 g \u548c 200 mg)\u6216\u5b89\u6170\u5242\uff0c\u6bcf\u65e53\u6b21\uff0c\u7597\u7a0b2\u5929\u3002\u4e3b\u8981\u9884\u540e\u6307\u6807\u4e3a\u65e9\u671f\u547c\u5438\u673a\u76f8\u5173\u6027\u80ba\u708e(\u5728\u4f4f\u9662\u6700\u521d7\u5929\u5185\u53d1\u751f)\u3002\u7531\u4e00\u4e2a\u72ec\u7acb\u7684\u8bc4\u4f30\u59d4\u5458\u4f1a\u786e\u5b9a\u547c\u5438\u673a\u76f8\u5173\u6027\u80ba\u708e\u7684\u8bca\u65ad\u3002<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">RESULTS \u7ed3\u679c<\/h2>\n\n\n\n<p>A total of 198 patients underwent randomization, and 194 were included in the analysis. After adjudication, 60 cases of ventilator-associated pneumonia were confirmed, including 51 of early ventilator-associated pneumonia. The incidence of early ventilator-associated pneumonia was lower with antibiotic prophylaxis than with placebo (19 patients [19%] vs. 32 [34%]; hazard ratio, 0.53; 95% confidence interval, 0.31 to 0.92; P=0.03). No significant differences between the antibiotic group and the control group were observed with respect to the incidence of late ventilator-associated pneumonia (4% and 5%, respectively), the number of ventilator-free days (21 days and 19 days), ICU length of stay (5 days and 8 days if patients were discharged and 7 days and 7 days if patients had died), and mortality at day 28 (41% and 37%). At day 7, no increase in resistant bacteria was identified.
