He reported a 4-month history of nonproductive cough, 5-kg weight loss, and fatigue. He immigrated to the United States as an adult more than 20 years before presentation. His temperature was 37<strong>\u00b0<\/strong>C, heart rate was 78\/min, respiratory rate was 17\/min, and blood pressure was 158\/95 mm Hg. A chest radiographic image suggested cardiomegaly and a computed tomographic scan demonstrated a moderate to large pericardial effusion. A pericardial drain was placed and pericardial fluid was sent to the laboratory for evaluation. Initial pericardial fluid study results are presented in the&nbsp;<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002t1\">Table<\/a>. Empirical treatment for tuberculosis was initiated. \u4e0b\u4e00\u6b65\u5e94\u8be5\u505a\u4ec0\u4e48\uff1f<\/strong><\/h2>\n\n\n\n<h2 class=\"wp-block-heading\">A: Stop further nontubercular diagnostic testing and continue antitubercular therapy \u505c\u6b62\u9488\u5bf9\u975e\u7ed3\u6838\u7684\u8fdb\u4e00\u6b65\u8bca\u65ad\u68c0\u67e5\uff0c\u7ee7\u7eed\u6297\u7ed3\u6838\u6cbb\u7597<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">Test Characteristics \u68c0\u67e5\u7279\u70b9<\/h3>\n\n\n\n<p>ADA is an enzyme in lymphocytes and myeloid cells that recycles toxic purine pathway metabolites, which are essential for DNA metabolism and cell viability.<sup><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r1\">1<\/a><\/sup><sup>,<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r2\">2<\/a><\/sup>\u00a0ADA levels are elevated in inflammatory effusions, including pleural, pericardial, and joint effusion, caused by bacterial infections, granulomatous inflammation (eg, tuberculosis, sarcoidosis), malignancy, and autoimmune diseases (eg, lupus, vasculitis).<sup><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r1\">1<\/a><\/sup><sup>,<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r2\">2<\/a><\/sup>\u00a0ADA is normally elevated in neutrophil-predominant effusions and is not a useful diagnostic test in the setting of neutrophil-predominant effusions.<sup><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r3\">3<\/a><\/sup>\u00a0However, among lymphocyte-predominant effusions, levels of ADA are typically higher in those caused by tuberculosis (TB) than those caused by other conditions.<sup><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r1\">1<\/a><\/sup><sup>,<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r2\">2<\/a><\/sup><\/p>\n\n\n\n<p>ADA\u662f\u6dcb\u5df4\u7ec6\u80de\u548c\u9ad3\u6837\u7ec6\u80de\u4e2d\u7684\u4e00\u79cd\u9176\uff0c\u53c2\u4e0e\u6bd2\u6027\u560c\u5464\u9014\u5f84\u4ee3\u8c22\u4ea7\u7269\u7684\u518d\u5faa\u73af\uff0c\u540e\u8005\u5bf9\u4e8eDNA\u4ee3\u8c22\u548c\u7ec6\u80de\u6d3b\u6027\u975e\u5e38\u91cd\u8981\u3002\u7ec6\u83cc\u611f\u67d3\u3001\u8089\u82bd\u80bf\u6027\u708e\u75c7\uff08\u5982\u7ed3\u6838\uff0c\u7ed3\u8282\u75c5\uff09\u3001\u6076\u6027\u80bf\u7624\u53ca\u81ea\u8eab\u514d\u75ab\u75c5\uff08\u5982\u72fc\u75ae\uff0c\u8840\u7ba1\u708e\uff09\u5f15\u8d77\u7684\u708e\u6027\u79ef\u6db2\uff08\u5305\u62ec\u80f8\u8154\u3001\u5fc3\u5305\u548c\u5173\u8282\u79ef\u6db2\uff09\u60c5\u51b5\u4e0bADA\u6c34\u5e73\u5347\u9ad8\u3002\u4ee5\u4e2d\u6027\u7c92\u7ec6\u80de\u4e3a\u4e3b\u7684\u79ef\u6db2ADA\u5347\u9ad8\uff0c\u6b64\u65f6ADA\u7684\u8bca\u65ad\u4ef7\u503c\u4e0d\u9ad8\u3002\u7136\u800c\uff0c\u5728\u4ee5\u6dcb\u5df4\u7ec6\u80de\u4e3a\u4e3b\u7684\u79ef\u6db2\u4e2d\uff0c\u7ed3\u6838(TB)\u60a3\u8005\u7684ADA\u6c34\u5e73\u901a\u5e38\u9ad8\u4e8e\u5176\u4ed6\u75c5\u56e0\u3002<\/p>\n\n\n\n<p>In lymphocyte-predominant effusions, the most common threshold for an ADA test result indicating TB is an ADA level greater than 40 U\/L. An ADA level greater than 40 U\/L has a sensitivity of 87% to 93% and specificity of 89% to 97% for TB.<sup><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r2\">2<\/a>,<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r4\">4<\/a><\/sup>\u00a0At a threshold of greater than 35 U\/L, sensitivity is higher (93%-95%) but specificity is lower (74%-90%).<sup><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r3\">3<\/a>,<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r5\">5<\/a>,<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r6\">6<\/a><\/sup>\u00a0Given a pretest probability of 70% for TB in a lymphocyte-predominant pericardial effusion in a patient from an endemic country,<sup><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r7\">7<\/a><\/sup>\u00a0an ADA level greater than 40 U\/L results in a posttest probability of 96%, while a level less than or equal to 40 U\/L results in a posttest probability of 19%. The Centers for Medicare &amp; Medicaid Services reimbursement rate for ADA testing is $8.10 (Current Procedural Terminology code 84311).<\/p>\n\n\n\n<p>\u5728\u6dcb\u5df4\u7ec6\u80de\u4e3a\u4e3b\u7684\u79ef\u6db2\u4e2d\uff0c\u63d0\u793a\u4e3aTB\u7684ADA\u4e34\u754c\u503c\u901a\u5e38\u4e3a40 U\/L\u3002ADA > 40 U\/L\u8bca\u65adTB\u7684\u654f\u611f\u602787% to 93%\uff0c\u7279\u5f02\u6027 89% to 97%\u3002\u4e34\u754c\u503c\u4e3a35 U\/L\u65f6\uff0c\u654f\u611f\u6027\u66f4\u9ad8(93%-95%)\uff0c\u4f46\u7279\u5f02\u6027\u8f83\u4f4e(74%-90%)\u3002\u5bf9\u4e8e\u4e00\u540d\u6765\u81ea\u6d41\u884c\u56fd\u5bb6\u7684\u60a3\u8005\uff0c\u5fc3\u5305\u79ef\u6db2\u4ee5\u6dcb\u5df4\u7ec6\u80de\u4e3a\u4e3b\uff0c\u8bca\u65adTB\u7684\u9a8c\u524d\u6982\u7387\u4e3a70%\u65f6\uff0c\u82e5ADA > 40 U\/L\uff0c\u5219\u9a8c\u540e\u6982\u7387\u4e3a96%\uff0c\u82e5ADA &lt;= 40 U\/L\uff0c\u5219\u9a8c\u540e\u6982\u7387\u4e3a 19%\u3002Medicare \u4e0e Medicaid \u670d\u52a1\u4e2d\u5fc3\u5bf9ADA\u68c0\u6d4b\u7684\u62a5\u9500\u4ef7\u683c\u4e3a $8.10\uff08\u5f53\u524d\u64cd\u4f5c\u540d\u8bcd\u7f16\u780184311\uff09\u3002<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Application to This Patient \u5728\u6b64\u4f8b\u60a3\u8005\u7684\u5e94\u7528<\/h3>\n\n\n\n<p>In the current patient, the ADA test was ordered from pericardial fluid to confirm the presumptive diagnosis of pericardial TB, for which antitubercular therapy was initiated. RIPE (rifampin, isoniazid, pyrazinamide, ethambutol) therapy was started because of the high pretest probability of TB in a patient from a country with endemic TB with chronic cough, weight loss, and lymphocyte-predominant pericardial effusion. The patient\u2019s ADA level of 118.1 U\/L confirmed the diagnosis, enabling cessation of further evaluation and supporting continuation of antitubercular therapy.<\/p>\n\n\n\n<p>\u5bf9\u4e8e\u6b64\u4f8b\u60a3\u8005\uff0c\u5fc3\u5305\u79ef\u6db2ADA\u68c0\u67e5\u786e\u8bca\u4e86\u5fc3\u5305TB\u7684\u521d\u59cb\u8bca\u65ad\u3002\u5bf9\u4e8e\u8fd9\u540d\u6765\u81eaTB\u6d41\u884c\u56fd\u5bb6\u7684\u957f\u671f\u54b3\u55fd\u3001\u4f53\u91cd\u4e0b\u964d\u4e14\u5fc3\u5305\u79ef\u6db2\u4ee5\u6dcb\u5df4\u7ec6\u80de\u4e3a\u4e3b\u7684\u60a3\u8005\uff0c\u7531\u4e8eTB\u7684\u9a8c\u524d\u6982\u7387\u5f88\u9ad8\uff0c\u56e0\u6b64\u5f00\u59cb\u4e86RIPE\uff08\u5229\u798f\u5e73\uff0c\u5f02\u70df\u80bc\uff0c\u5421\u55ea\u9170\u80fa\uff0c\u4e59\u80fa\u4e01\u9187\uff09\u6cbb\u7597\u3002\u60a3\u8005ADA\u6c34\u5e73118.1 U\/L\u660e\u786e\u4e86\u8bca\u65ad\uff0c\u53ef\u4ee5\u505c\u6b62\u8fdb\u4e00\u6b65\u68c0\u67e5\uff0c\u5e76\u652f\u6301\u7ee7\u7eed\u6297\u7ed3\u6838\u6cbb\u7597\u3002<\/p>\n\n\n\n<p>An alternative approach is to await a culture positive for TB before starting therapy. However, cultures positive for TB can take weeks to grow, which can allow infection to progress while awaiting confirmation. An ADA level greater than 40 U\/L in a patient with a high pretest probability of disease results in a sufficiently high posttest probability to justify a full course of TB therapy. Furthermore, even with an ADA level less than or equal to 40 U\/L in a patient with a high pretest probability, the posttest probability of approximately 20% is sufficiently high to warrant empirical therapy. However, the low ADA level would necessitate further diagnostic testing, including invasive pericardial biopsy and serologies, to rule out other causes (eg, sarcoid, malignancy, vasculitis) while treating the patient for TB. In that situation, empirical TB therapy could have been stopped if an alternative diagnosis was established.<\/p>\n\n\n\n<p>\u53e6\u4e00\u79cd\u7b56\u7565\u662f\u7b49\u5f85TB\u57f9\u517b\u7ed3\u679c\u9633\u6027\u540e\u518d\u5f00\u59cb\u6cbb\u7597\u3002\u7136\u800c\uff0cTB\u57f9\u517b\u9700\u8981\u6570\u5468\u624d\u80fd\u5f97\u5230\u7ed3\u679c\uff0c\u5728\u6b64\u671f\u95f4\u611f\u67d3\u53ef\u80fd\u8fdb\u5c55\u3002\u5bf9\u4e8e\u9a8c\u524d\u6982\u7387\u5f88\u9ad8\u7684\u60a3\u8005\uff0cADA > 40 U\/L\u80fd\u591f\u786e\u4fdd\u8db3\u591f\u9ad8\u7684\u9a8c\u540e\u6982\u7387\uff0c\u53ef\u4ee5\u8fdb\u884c\u6297\u7ed3\u6838\u6cbb\u7597\u3002\u800c\u4e14\uff0c\u5373\u4f7f\u9a8c\u524d\u6982\u7387\u5f88\u9ad8\u7684\u60a3\u8005ADA &lt;= 40 U\/L\uff0c\u9a8c\u540e\u6982\u7387\u4ecd\u63a5\u8fd120%\uff0c\u6b64\u65f6\u4ecd\u53ef\u8fdb\u884c\u7ecf\u9a8c\u6027\u6cbb\u7597\u3002\u4f46\u662f\uff0c\u5982\u679cADA\u6c34\u5e73\u8f83\u4f4e\uff0c\u5728\u8fdb\u884c\u6297\u7ed3\u6838\u6cbb\u7597\u7684\u540c\u65f6\uff0c\u9700\u8981\u8fdb\u4e00\u6b65\u8bca\u65ad\u68c0\u67e5\uff0c\u5305\u62ec\u5fc3\u5305\u6d3b\u68c0\u548c\u8840\u6e05\u5b66\u68c0\u67e5\uff0c\u4ee5\u6392\u9664\u5176\u4ed6\u75c5\u56e0\uff08\u5982\u7ed3\u8282\u75c5\uff0c\u6076\u6027\u80bf\u7624\u548c\u8840\u7ba1\u708e\uff09\u3002\u5728\u8fd9\u79cd\u60c5\u51b5\u4e0b\uff0c\u5982\u679c\u786e\u5b9a\u5176\u4ed6\u8bca\u65ad\uff0c\u5219\u5e94\u5f53\u505c\u6b62\u7ecf\u9a8c\u6027\u6297\u7ed3\u6838\u6cbb\u7597\u3002<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">What Are Alternative Diagnostic Testing Approaches? \u662f\u5426\u6709\u5176\u4ed6\u7684\u8bca\u65ad\u65b9\u6cd5\uff1f<\/h3>\n\n\n\n<p>Elevated interferon-\u03b3 concentrations in effusions have high sensitivity (95.7%) and specificity (96.3%) for active TB, but their use is limited by cost and availability.<sup><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r1\">1<\/a>,<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r4\">4<\/a><\/sup>\u00a0Interferon-\u03b3 can also be measured from whole blood via IGRAs. However, IGRAs and the similar purified protein derivative skin test are only useful to diagnose latent TB. Laboratory test results indicating ADA values greater than 40 U\/L cannot distinguish between latent and active TB (nonspecific), and IGRAs\/purified protein derivatives are associated with false-negative results in up to 30% of patients with pulmonary TB and 50% of patients with disseminated TB,<sup><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r8\">8<\/a>,<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2736434#jdt190002r9\">9<\/a><\/sup>\u00a0because active TB causes antigen-specific anergy of T cells.<\/p>\n\n\n\n<p>\u79ef\u6db2\u4e2d\u03b3\u5e72\u6270\u7d20\u6c34\u5e73\u5347\u9ad8\u5bf9\u4e8e\u6d3b\u52a8\u6027TB\u5177\u6709\u5f88\u9ad8\u7684\u654f\u611f\u6027(95.7%)\u548c\u7279\u5f02\u6027(96.3%)\uff0c\u4f46\u7531\u4e8e\u8d39\u7528\u53ca\u68c0\u67e5\u666e\u53ca\u6027\u9650\u5236\u4e86\u4e34\u5e8a\u5e94\u7528\u3002\u8fd8\u53ef\u4ee5\u5bf9\u5168\u8840\u901a\u8fc7IGRA\u65b9\u6cd5\u6d4b\u5b9a\u03b3\u5e72\u6270\u7d20\u3002\u7136\u800c\uff0cIGRA\u53ca\u76f8\u4f3c\u7684\u7eaf\u5316\u86cb\u767d\u884d\u751f\u7269(PPD)\u76ae\u8bd5\u4ec5\u7528\u4e8e\u8bca\u65ad\u6f5c\u4f0f\u671fTB\u3002\u5b9e\u9a8c\u5ba4\u68c0\u67e5\u63d0\u793aADA > 40 U\/L\uff0c\u5e76\u4e0d\u80fd\u9274\u522b\u6f5c\u4f0f\u671f\u548c\u6d3b\u52a8\u671fTB\uff08\u7f3a\u4e4f\u7279\u5f02\u6027\uff09\uff0c\u80baTB\u60a3\u8005IGRA\/PPD\u5047\u9634\u6027\u7387\u53ef\u8fbe30%\uff0c\u64ad\u6563\u6027TB\u60a3\u8005\u5047\u9634\u6027\u738750% \uff0c\u8fd9\u662f\u7531\u4e8e\u6d3b\u52a8\u671fTB\u53ef\u5f15\u8d77T\u7ec6\u80de\u6297\u539f\u7279\u5f02\u6027\u65e0\u53cd\u5e94\u3002<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Patient Outcomes \u60a3\u8005\u7ed3\u5c40<\/h3>\n\n\n\n<p>The patient\u2019s sputum was negative for acid-fast bacilli smears and TB polymerase chain reaction. Pericardial fluid cytology demonstrated inflammatory cells, but was negative for malignant cells, acid-fast bacilli stain, and TB polymerase chain reaction.
