Russell, et al\u00a0<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">N Engl J Med February 18, 2019<br \/>DOI: 10.1056\/NEJMoa1812405<\/h3>\n\n\n\n<h2 class=\"wp-block-heading\">BACKGROUND \u80cc\u666f<\/h2>\n\n\n\n<p>Hypoxemia is the most common complication during tracheal intubation of critically ill adults and may increase the risk of cardiac arrest and death. Whether positive-pressure ventilation with a bag-mask device (bag-mask ventilation) during tracheal intubation of critically ill adults prevents hypoxemia without increasing the risk of aspiration remains controversial.<\/p>\n\n\n\n<p>\u4f4e\u6c27\u8840\u75c7\u662f\u6210\u5e74\u5371\u91cd\u75c5\u60a3\u8005\u6c14\u7ba1\u63d2\u7ba1\u8fc7\u7a0b\u4e2d\u6700\u5e38\u89c1\u7684\u5e76\u53d1\u75c7\uff0c\u53ef\u80fd\u589e\u52a0\u5fc3\u8df3\u9aa4\u505c\u51e0\u6b7b\u4ea1\u98ce\u9669\u3002\u6210\u5e74\u5371\u91cd\u75c5\u60a3\u8005\u6c14\u7ba1\u63d2\u7ba1\u8fc7\u7a0b\u4e2d\uff0c\u901a\u8fc7\u7403\u56ca\u9762\u7f69\u88c5\u7f6e\u8fdb\u884c\u6b63\u538b\u901a\u6c14\u80fd\u5426\u9884\u9632\u4f4e\u6c27\u8840\u75c7\u4e14\u4e0d\u589e\u52a0\u8bef\u5438\u98ce\u9669\u5c1a\u5b58\u5728\u4e89\u8bae\u3002<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">METHODS \u65b9\u6cd5<\/h2>\n\n\n\n<p>In a multicenter, randomized trial conducted in seven intensive care units in the United States, we randomly assigned adults undergoing tracheal intubation to receive either ventilation with a bag-mask device or no ventilation between induction and laryngoscopy. The primary outcome was the lowest oxygen saturation observed during the interval between induction and 2 minutes after tracheal intubation. The secondary outcome was the incidence of severe hypoxemia, defined as an oxygen saturation of less than 80%.<\/p>\n\n\n\n<p>\u8fd9\u662f\u5728\u7f8e\u56fd7\u4e2aICU\u8fdb\u884c\u7684\u4e00\u9879\u591a\u4e2d\u5fc3\u968f\u673a\u4e34\u5e8a\u8bd5\u9a8c\u3002\u6211\u4eec\u5c06\u63a5\u53d7\u6c14\u7ba1\u63d2\u7ba1\u7684\u6210\u5e74\u60a3\u8005\u968f\u673a\u5206\u7ec4\uff0c\u5728\u8bf1\u5bfc\u53ca\u5589\u955c\u64cd\u4f5c\u9636\u6bb5\u5206\u522b\u63a5\u53d7\u7403\u56ca\u9762\u7f69\u901a\u6c14\u6216\u65e0\u901a\u6c14\u3002\u4e3b\u8981\u9884\u540e\u7ec8\u70b9\u4e3a\u8bf1\u5bfc\u81f3\u6c14\u7ba1\u63d2\u7ba1\u540e2\u5206\u949f\u5185\u7684\u6700\u4f4e\u6c27\u9971\u548c\u5ea6\u3002\u6b21\u8981\u9884\u540e\u7ec8\u70b9\u4e3a\u4e25\u91cd\u4f4e\u6c27\u8840\u75c7\u53d1\u751f\u7387\uff0c\u5b9a\u4e49\u4e3a\u6c27\u9971\u548c\u5ea6&lt; 80%\u3002<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">RESULTS \u7ed3\u679c<\/h2>\n\n\n\n<p>Among the 401 patients enrolled, the median lowest oxygen saturation was 96% (interquartile range, 87 to 99) in the bag-mask ventilation group and 93% (interquartile range, 81 to 99) in the no-ventilation group (P=0.01). A total of 21 patients (10.9%) in the bag-mask ventilation group had severe hypoxemia, as compared with 45 patients (22.8%) in the no-ventilation group (relative risk, 0.48; 95% confidence interval [CI], 0.30 to 0.77). Operator-reported aspiration occurred during 2.5% of intubations in the bag-mask ventilation group and during 4.0% in the no-ventilation group (P=0.41).
