We aimed to investigate the long-term impact on physical and neurocognitive development of early versus late parenteral nutrition.<\/p>\n\n\n\n<p>\u513f\u7ae5\u5371\u91cd\u75c5\u65e9\u671f\u4e0e\u665a\u671f\u80c3\u80a0\u5916\u8425\u517b\u7814\u7a76(PEPaNIC)\u662f\u4e00\u9879\u591a\u4e2d\u5fc3\u968f\u673a\u5bf9\u7167\u7814\u7a76\uff0c\u7ed3\u679c\u663e\u793a\uff0c\u4e0e\u65e9\u671f\u80c3\u80a0\u5916\u8425\u517b\u76f8\u6bd4\uff0c\u5728\u513f\u79d1ICU (PICU)\u9650\u5236\u80c3\u80a0\u5916\u8425\u517b\u4e00\u5468\uff08\u665a\u671f\u80c3\u80a0\u5916\u8425\u517b\uff09\u80fd\u591f\u51cf\u5c11\u5371\u91cd\u75c5\u60a3\u513f\u7684\u611f\u67d3\uff0c\u5e76\u52a0\u901f\u5eb7\u590d\u3002\u6211\u4eec\u65e8\u5728\u7814\u7a76\u65e9\u671f\u53ca\u665a\u671f\u80c3\u80a0\u5916\u8425\u517b\u5bf9\u8eab\u4f53\u53ca\u795e\u7ecf\u8ba4\u77e5\u53d1\u80b2\u7684\u8fdc\u671f\u5f71\u54cd\u3002<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Methods \u65b9\u6cd5<\/h2>\n\n\n\n<p>In this preplanned 2-year follow-up study, all patients included in the PEPaNIC trial (which was done in University Hospitals Leuven, Belgium; Erasmus Medical Centre\u2013Sophia Children's Hospital, Rotterdam, Netherlands; and Stollery Children's Hospital, Edmonton, AB, Canada) were approached for possible assessment of physical and neurocognitive development compared with healthy children who were matched for age and sex, and who had never been admitted to a neonatal ICU or a PICU. Assessed outcomes comprised anthropometric data; health status; parent-reported or caregiver-reported executive functions and emotional and behavioural problems; and tests for intelligence, visual\u2013motor integration, alertness, motor coordination, inhibitory control, cognitive flexibility, and memory. To address partial responses among the children tested, we did multiple data imputation by chained equations before univariable and multivariable linear and logistic regression analyses adjusted for risk factors. This trial is registered with&nbsp;<a rel=\"noreferrer noopener\" href=\"http:\/\/clinicaltrials.gov\/\" target=\"_blank\">ClinicalTrials.gov<\/a>, number&nbsp;<a rel=\"noreferrer noopener\" href=\"http:\/\/clinicaltrials.gov\/show\/NCT01536275\" target=\"_blank\">NCT01536275<\/a>.<\/p>\n\n\n\n<p>\u5728\u8fd9\u9879\u9884\u5148\u8ba1\u5212\u76842\u5e74\u968f\u8bbf\u7814\u7a76\u4e2d\uff0c\u6240\u6709\u7eb3\u5165PEPaNIC\u7814\u7a76\uff08\u5728\u6bd4\u5229\u65f6Leuven\u5927\u5b66\u533b\u9662\uff0c\u8377\u5170\u9e7f\u7279\u4e39Erasmus\u533b\u5b66\u4e2d\u5fc3-Sophia\u513f\u7ae5\u533b\u9662\uff0c\u53ca\u52a0\u62ff\u5927\u57c3\u5fb7\u8499\u987fStollery\u513f\u7ae5\u533b\u9662\uff09\u7684\u60a3\u8005\u5747\u63a5\u53d7\u8eab\u4f53\u53ca\u795e\u7ecf\u8ba4\u77e5\u53d1\u80b2\u8bc4\u4f30\uff0c\u5e76\u4e0e\u5e74\u9f84\u6027\u522b\u76f8\u5339\u914d\u7684\u5065\u5eb7\u513f\u7ae5\uff08\u4ece\u672a\u6536\u5165\u65b0\u751f\u513fICU\u6216PICU\uff09\u8fdb\u884c\u6bd4\u8f83\u3002\u8bc4\u4f30\u9884\u540e\u6307\u6807\u5305\u62ec\u4eba\u4f53\u6d4b\u91cf\u6570\u636e\uff1b\u5065\u5eb7\u72b6\u51b5\uff1b\u5bb6\u957f\u6216\u7167\u62a4\u4eba\u5458\u62a5\u544a\u7684\u6267\u884c\u529f\u80fd\u53ca\u60c5\u7eea\u548c\u884c\u4e3a\u95ee\u9898\uff1b\u4ee5\u53ca\u667a\u529b\u3001\u89c6\u89c9\u8fd0\u52a8\u6574\u5408\u4e0e\u8bb0\u5fc6\u529b\u6d4b\u8bd5\u7ed3\u679c\u3002\u4e3a\u89e3\u51b3\u6d4b\u8bd5\u513f\u7ae5\u90e8\u5206\u53cd\u5e94\u7684\u95ee\u9898\uff0c\u6211\u4eec\u5728\u5355\u56e0\u7d20\u53ca\u591a\u56e0\u7d20\u7ebf\u6027\u548clogistic\u56de\u5f52\u5206\u6790\u5bf9\u5371\u9669\u56e0\u7d20\u8fdb\u884c\u6821\u6b63\u524d\uff0c\u901a\u8fc7\u94fe\u5f0f\u65b9\u7a0b\u8fdb\u884c\u591a\u91cd\u6570\u636e\u5dee\u8865\u3002\u8bd5\u9a8c\u5728<a rel=\"noreferrer noopener\" href=\"http:\/\/clinicaltrials.gov\/\" target=\"_blank\">ClinicalTrials.gov<\/a>\u6ce8\u518c\uff0c\u6ce8\u518c\u53f7<a rel=\"noreferrer noopener\" href=\"http:\/\/clinicaltrials.gov\/show\/NCT01536275\" target=\"_blank\">NCT01536275<\/a>\u3002<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Findings \u7ed3\u679c<\/h2>\n\n\n\n<p>At the 2-year follow-up, 60 (8%) of 717 children who received late parenteral nutrition and 63 (9%) of 723 children who received early parenteral nutrition had died (p=0\u00b781). 68 (9%) of 717 children who received late and 91 (13%) of 723 children who received early parenteral nutrition were too disabled for neurocognitive assessment (p=0\u00b7059), and 786 patients (395 assigned to late and 391 assigned to early parenteral nutrition) consented for testing. 786 patients and 405 healthy control children underwent long-term outcome testing between Aug 4, 2014, and Jan 19, 2018, and were included in the imputation model for subsequent multivariable analyses. Late parenteral nutrition did not adversely affect anthropometric data, health status, or neurological functioning, and improved parent-reported or caregiver-reported executive functioning (late&nbsp;<em>vs<\/em>&nbsp;early parenteral nutrition \u03b2 estimate \u22122\u00b7258, 95% CI \u22124\u00b7012 to \u22120\u00b7504; p=0\u00b7011), more specifically inhibition (\u22123\u00b7422, \u22125\u00b7171 to \u22121\u00b7673; p=0\u00b70001), working memory (\u22122\u00b7016, \u22123\u00b7761 to \u22120\u00b7270; p=0\u00b7023), and meta-cognition (\u22121\u00b7957, \u22123\u00b7694 to \u22120\u00b7220; p=0\u00b7027). Externalising behavioural problems (\u03b2 estimate \u22121\u00b7715, 95% CI \u22123\u00b7325 to \u22120\u00b7106; p=0\u00b7036) and visual\u2013motor integration (0\u00b7468, 0\u00b7087 to 0\u00b7850; p=0\u00b7016) were also improved in the late parenteral nutrition group compared with the early parenteral nutrition group. After Bonferroni correction for multiple comparisons, the effect on inhibitory control remained significant (p=0\u00b70001).<\/p>\n\n\n\n<p>\u57282\u5e74\u968f\u8bbf\u65f6\uff0c\u665a\u671f\u80c3\u80a0\u5916\u8425\u517b\u7ec4717\u540d\u60a3\u513f\u4e2d60\u540d(8%)\uff0c\u4ee5\u53ca\u65e9\u671f\u80c3\u80a0\u5916\u8425\u517b\u7ec4723\u540d\u60a3\u8005\u4e2d63\u540d(9%)\u6b7b\u4ea1(p=0\u00b781)\u3002\u665a\u671f\u80c3\u80a0\u5916\u8425\u517b\u7ec4717\u540d\u60a3\u513f\u4e2d68\u540d(9%)\u53ca\u65e9\u671f\u80c3\u80a0\u5916\u8425\u517b\u7ec4723\u540d\u60a3\u8005\u4e2d91\u540d(13%)\u56e0\u75c5\u65e0\u6cd5\u63a5\u53d7\u795e\u7ecf\u8ba4\u77e5\u529f\u80fd\u8bc4\u4f30(p=0\u00b7059)\uff0c\u5171\u6709786\u540d\u60a3\u8005\uff08\u665a\u671f\u7ec4395\u540d\uff0c\u65e9\u671f\u7ec4391\u540d\uff09\u540c\u610f\u63a5\u53d7\u6d4b\u8bd5\u3002\u57282014\u5e748\u67084\u65e5\u81f32018\u5e741\u670819\u65e5\u4e4b\u95f4\uff0c786\u540d\u60a3\u8005\u4e0e405\u540d\u5065\u5eb7\u513f\u7ae5\u63a5\u53d7\u8fdc\u671f\u9884\u540e\u6d4b\u8bd5\uff0c\u5e76\u7eb3\u5165\u540e\u7eed\u591a\u56e0\u7d20\u5206\u6790\u7684\u63d2\u8865\u6a21\u578b\u3002\u665a\u671f\u80c3\u80a0\u5916\u8425\u517b\u5e76\u4e0d\u5bf9\u4eba\u53e3\u6d4b\u91cf\u5b66\u6307\u6807\uff0c\u5065\u5eb7\u72b6\u51b5\u6216\u795e\u7ecf\u7cfb\u7edf\u529f\u80fd\u4ea7\u751f\u8d1f\u9762\u5f71\u54cd\uff0c\u4f46\u5bb6\u957f\u6216\u7167\u62a4\u8005\u62a5\u544a\u7684\u6267\u884c\u529f\u80fd\u6539\u5584\uff08\u665a\u671f\u4e0e\u65e9\u671f\u80c3\u80a0\u5916\u8425\u517b\u03b2\u503c\u22122\u00b7258, 95% CI \u22124\u00b7012 to \u22120\u00b7504; p=0\u00b7011\uff09\uff0c\u7279\u5f02\u6027\u6291\u5236\u66f4\u591a(\u22123\u00b7422, \u22125\u00b7171 to \u22121\u00b7673; p=0\u00b70001)\uff0c\u5de5\u4f5c\u8bb0\u5fc6\u66f4\u591a(\u22122\u00b7016, \u22123\u00b7761 to \u22120\u00b7270; p=0\u00b7023)\uff0c\u540e\u8bbe\u8ba4\u77e5\u66f4\u591a(\u22121\u00b7957, \u22123\u00b7694 to \u22120\u00b7220; p=0\u00b7027)\u3002\u4e0e\u65e9\u671f\u80c3\u80a0\u5916\u8425\u517b\u7ec4\u76f8\u6bd4\uff0c\u665a\u671f\u80c3\u80a0\u5916\u8425\u517b\u7ec4\u5916\u5728\u5316\u884c\u4e3a\u95ee\u9898\uff08\u03b2\u503c\u22121\u00b7715, 95% CI \u22123\u00b7325 to \u22120\u00b7106;
