Dear Colleagues,
On behalf of the investigators of the Cuff leak and airway Obstruction in Mechanically ventilated ICU patients (COMIC) Pilot Trial, we invite you to complete an anonymous survey regarding the use of the cuff leak test (CLT) as a screening tool for laryngeal edema prior to extubation.
The CLT is used to screen for potential airway edema prior to extubation. A failed CLT implies that there may be laryngeal edema while a passed test implies no laryngeal edema. There are conflicting results on the actual utility and accuracy of a CLT. While one meta-analysis of observational studies found that the CLT was a specific predictor of reintubation, another meta-analysis found no such association. Recent guidelines reflect this uncertainty and a weak recommendation was issued for performing CLT in mechanically ventilated patients who are at a high risk of postextubation stridor. Data on attitude and beliefs of intensivists with regards to performing CLT are lacking.
